Well Child Schedule

Age

 

· Newborn Visit at Hospital

· 3-5 days old

· 10-14 days old

· 2 months old

· 4 months old

· 6 months old

· 9 months old

· 12 months old

· 18 months old

· 2 years - 6 years—every year

· Over 6 years old—every 2 years, annual exam if you prefer

 

We are happy to see your child through his or her college years.

We follow the American Academy of Pediatrics recommendations for routine health maintenance examinations.

 

During these visits, we will weigh and measure your child, assess other parameters as appropriate including vision, hearing, blood pressure, etc. Age-appropriate history is obtained, a developmental assessment is done when appropriate, and you are given an opportunity to ask questions. A head-to-toe physical examination is then done. Afterward, your child’s history and physical examination are discussed. Safety and anticipatory guidance issues may be covered, and a review of immunizations is done.

 

Please have your older children wear or bring a T-shirt and gym shorts to their exam for their comfort.

 

As your child becomes verbal, talk with him or her about their upcoming exam. As they get older, have them think about the exam, consider any questions they may have for the doctor, and encourage them to discuss any concerns with you prior to the exam.

 

You are probably the best judge as to whether you should tell your child if they will be having immunizations—my opinion (HCQ) is not to tell unless asked. Be cautious not to assure them that there won’t be shots—vaccine schedules sometimes change.

 

If you have sports or camp forms, please complete your portion of the form prior to the visit.

 

Please keep a copy of your child’s immunization record in a safe place at home for your reference. Bring your immunization card in with you to each physical examination so that it may be kept current.”
